III. CUSTOMER BENEFITS

We may request additional financial information in order to provide you credit. In addition, we may request information that helps us keep your account secure. We use the information you give us to optimize the financial services we offer, evaluate your creditworthiness and prevent fraud. We will not be able to process your application if you do not provide us with the information requested on the application.

As a part of an application for credit or the opening of an account, you expressly authorize us to obtain a credit bureau report for our use in evaluating your application and subsequently in connection with our periodic review of your account relationship(s) with us, a requested change to your account(s) or a change in your credit line or renewal of your account. VI. HOW FIRST BANK USES PERSONAL INFORMATION

A. To Evaluate Your Request
FB uses your personal information to evaluate your request to open an account or to determine whether to issue credit and, if we extend an offer of credit, to establish your line of credit. Thereafter, we use the information to provide our clients with the highest level of service and protect our customers from fraudulent use of their account. In certain circumstances products may be offered through FB and provided by an unaffiliated company on behalf of FB. In these instances, the application information will be forwarded to the provider company for processing.

B. To Provide Products that Meet Customer Needs
FB is always looking for opportunities to provide value to our customers by making services available that the customers may need. When we identify an opportunity we may communicate with customers who may be interested in the services in these ways:

Monthly Statements
Direct Mail, e-mails or Telemarketing

Third-party processors or providers may conduct the actual sending of the offer. We require that these third parties hold the information we provide in the strictest confidence. The third party cannot use the list for any purpose other than the particular offer of goods or services for which information has been provided.

C. Use of Credit Bureaus
Like most, (if not all banks), FB shares our experience with you with the credit bureaus. Our reporting to credit bureaus is governed by the Fair Credit Reporting Act. Under the Fair Credit Reporting Act you have the right to make sure that your credit bureau reports are correct and accurate.

D. Lawful Request by a Court, Government or Regulatory Agency
We will disclose your personal information in response to a lawful request issued by a court, government agency, or regulatory authority with jurisdiction to make the request.

X. ACCESS TO MY INFORMATION/QUESTIONS/E-MAIL

If you believe there is an error or you have a question about your account, write us at: First Bank of Jacksonville, 11100 San Jose Blvd., Jacksonville, FL 32223 Attn: Customer Service. Write to us as soon as possible after you become aware of the suspected problem. We must hear from you no later than 60 days after we sent you the first bill or statement on which the error or problem appeared. You can telephone us, but doing so will not preserve you rights.

In your letter give us the following information:

Your name and account number
The dollar amount of the suspected error
Describe the error and explain, if you can, why you believe there is an error

If you send us an e-mail, we have established an easy and secure method to send and receive e-mail. Please visit our Internet Banking site to use secure messaging to communicate with us.

Secure Communications
Our system ensures that data exchanged between your PC and our computers are encrypted with 128-bit encryption. Encryption is accomplished through Secure Sockets Layers (SSL), which utilizes mathematical formulas to ‘encrypt’ information on the Internet. Additionally, if SSL detects that data was added or deleted after you sent it to the bank, the connection will be severed in order to guard against any tampering.

Secure Environment
Our computer system does not connect directly to the Internet. It is isolated from the network through a firewall. A firewall is a combination of software and hardware products that designate parameters, and control and limit the access that outside computers have to the banks’ internal networks and databases.

Secure Login
User IDs and passwords are isolated from the Internet by being stored on First Bank’s computer. This way, the passwords and IDs are not accessed or downloaded by anyone on the Internet. The system also checks for invalid logins and automatically disallows a user after several invalid attempts.

Your Password
You will be responsible for maintaining your password. When you receive your Online Banking password, be sure to keep it confidential. Memorize it, or write it down and keep it in a safe place. It is not recommended you carry your password in your wallet. Make sure no one watches you enter your password and always close your browser when leaving the computer. Make sure you regularly scan your computer for viruses that could be used to capture your password.
